Notice of Upcoming Changes
Greetings from Blueprint Counseling Solutions! We had an amazing second (2nd) year of business with all thanks given to our amazing clients. As we continue to grow and be successful, there will be some important changes in operations. Please review the following items and feel free to discuss your concerns or questions with Latisha H. Joseph, LMFT at your next appointment. We are very thankful for you all and look forward to continuing our working relationship.
EFFECTIVE JANUARY 1, 2020:
Service Locations: Our primary office location is in Mission Valley at 1455 Frazee Road, Suite 500, San Diego, CA 92108.
We will also offer Telehealth services (Video only) provided via the Simple Practice Telehealth App available to download on the Google Play or Apple Store. In order to use telehealth services, you need to sign a Telehealth Consent Form and complete a Telehealth Crisis Protocol Form before your first video session.
Scheduling: You now have the option to schedule or modify your appointments on your client portal. If you don’t have access to this portal, please let us know and we will send you an invite. You must have an active email address to access the portal. The portal enables you to: receive billing statements, make payments, schedule and modify appointments, complete requested paperwork, upload documents, and send secure messages. https://blueprintmylife.clientsecure.me
*Please note: If you do not participate in therapeutic services for a period of 30 days or more, your case will be closed. Therapy is beneficial when you are consistent, therefore, we would prefer you commit to services consistently so you can get the results you need.
Billing: Fees are payable at the time that services are rendered. If payment is not received, you will receive an invoice. Please ask me if you wish to discuss a written agreement that specifies an alternative payment procedure. [This does not apply to clients utilizing health insurance unless the fee is for a late cancellation, no-show, or other services].
Appointments must be cancelled 24 hours in advance excluding holidays and weekends. For instance; if calling to cancel for a Monday appointment, the cancellation must be made by Friday. If cancelling over a holiday, the holiday itself does not count as normal business hours. In the case of a late cancellation, a fee of $25.00 will be made directly to the office by the client. In the case of a No-Show appointment, a fee of $50.00 will be made directly to the office by the client. Due to the volume of clients in need of services, each appointment is not only imperative but valuable to both you as the client and to the provider to maintain the efficiency of the practice. Exceptions may be made if you are sick or have an unavoidable emergency. (*Please note: You will receive one (1) excused absence without penalty with a verbal warning. After three (3) missed appointments, you may be subject to removal from the calendar).
Letters of Support: Due to the increase in clients working with Blueprint Counseling Solutions, there is a higher demand for letters of support that takes a considerable amount of time to complete. In addition, there needs to be adequate assessments and treatment planning addressed before a comprehensive and accurate letter of support can be provided to clients. The cost for any type of supporting documentation (excluding your initial assessment, treatment plans, discharge summary, secondary authorization request) will be $25.00 per additional item. The following process will now be in place:
Disability Support Letters (VA/ SSDI/ ETC.):
Must be a current client of Latisha H. Joseph, LMFT
Submit a written request detailing the purpose of the letter and information that needs to be included. Requests will be answered and letters provided within 2 weeks of the formal request.
Must have completed a minimum of 12 face-to-face sessions to include initial assessment and treatment plan.
A payment of $25 paid in full. (If you need a medical record review, it will cost an additional $50.00/ hour to be paid in full for all services provided).
Emotional Support Animal (ESA) Support Letters:
Must complete a face-to-face evaluation with support animal, in attendance.
Current Clients: Must complete a 45-60 minute face-to-face evaluation with support animal in attendance. This evaluation will be priced at your regular session cost, with an additional fee of $25.00 for the ESA letter.
Fees are due at the time of service.
